Casco
Documents required for the notification and settlement of motor hull Casco losses:
- Compensation Claim Form verified by the Insurance Company

- Power of attorney for driving a motor vehicle if the insured / claimant is a legal party or the owner of the vehicle is a leasing company
- Statements by the participants in the traffic accident

Green Card Insurance
What to do in case of a car accident abroad
- Do not remove the vehicle from the road
- Place an appropriate warning sign for other traffic participants
- Take photos of the accident site and the condition of the vehicles
- Fill in a European Accident Report in the case of a minor accident. This template has a unified content in all European countries, so the Macedonian sample will be your guide on how to fill it in.
- In case of an accident with a severe material damage, bodily or fatal /death consequences, be sure to report this event to the police to inspect the site and make a police report.
- Exchange copies of green cards with the other participant and / or give them to the police
- Request a note from the police
- Fill in the report if the police require that from you and be objective
- Damage reporting procedure
- If you are not responsible for the traffic accident and the damage is caused by an insured vehicle report the damage to the Bureau in the country where the accident occurred. The phone numbers and addresses of the green card bureaus are listed at the back of the green card.
- If the damage is caused by an uninsured vehicle (irrespectively whether it is a vehicle with a registration plate of the country visited or another foreign vehicle with a registration plate from a member state of the Council of Green Cards Bureaux) the damage is reported to the Guarantee Fund in the country where the accident occurred.
- In case you are responsible for the accident, you must notify the insurance company that issued the green card for all relevant events related to the accident, immediately upon arrival in the North Republic of Macedonia.
Property
Documents required for claiming damages based on the Property Insurance Policy:
- Compensation Claim Form verified by the Insurance Company
- A photocopy of property insurance policy
- Repair invoices and payment receipts after repairs
- Document for an acquisition cost of the damaged property

Travel Insurance
Documents required for claiming damages based on Travel Insurance Policy :
- Compensation Claim Form verified by the Insurance Company
- A photocopy of travel insurance policy
- Passport copy
- Airline tickets
- Luggage or flight delay report
- Original medical documentation (report) with diagnosis and detailed medical treatment Description
- Complete history of the patient's disease and type of prescribed therapy (medication)
- Original invoices / treatment confirmation with the stamp and signature of an authorized doctor or pharmacist
- An official report from the police on the type of accident, where the insured's injury is a result of an accident

Transportation Insurance
Documents required for claiming damages based on the Transportation Insurance Policy:
- Compensation Claim Form verified by the Insurance Company
- A photocopy of transportation insurance policy
- Invoice value of the goods being transported with a duly stated departure date including place and direction of transport
- Copy of registration/termination of employment M1 and M2 (COPY) for the driver of the vehicle to prove that he is an employee of the insured

CMR (Convention on the Contract for the International Carriage of Goods by Road)
Documents required for claiming damages:
- Compensation Claim Form verified by the Insurance Company
- Damage compensation claim of the injured party
- Transport documents (waybill, specification)
- Customs declaration form or invoice value of the goods
- Driver's written statement of the cause of the damage
- Emergency Loss Assessment Report
- Evidence on which it can be ascertained whether the insured party may or may not be held liable for the damage caused
- Police report, especially for losses resulting from traffic accident, fire or robbery
Accident Insurance Policy
Documents required for claiming damages based on the Accident Insurance Policy:
- Compensation Claim Form verified by the Insurance Company
- A photocopy of the accident insurance policy
- Employment Certificate stating the exact number of the policy
- Medical documentation
- Additional documentation that may be in the interest of determining the exact percentage of disability, as well as calculating additional treatment and subsistence costs provided they are covered by the collective insurance policy
- Medical Certificate for Sick Leave
- Original medical expenses bills
- Birth certificate (for minors - pupils, kindergarten children, etc.)
- A photocopy of the transaction account of the injured party
Liability Insurance
Documents required for claiming damages based on the Liability Insurance Policy:
- Compensation Claim Form verified by the Insurance Company
- A photocopy of liability insurance policy
- Damage Assessment Form
Traffic accident
PROCEDURE IN CASE OF A TRAFFIC ACCIDENT:
- Secure and mark the road accident location with a safety triangle
- If there are dead or injured, it is obligatory to call an ambulance dialling the telephone number 194
- You should call the police immediately on the phone 192 in the following cases:
- when a significant (large) material damage was caused
- when one of the vehicles has no registration plates or is not insured
- when one of the vehicles has a foreign registration plate
- when one of the drivers is under influence of alcohol or narcotics
- when the drivers cannot harmonize their opinions regarding the way and the circumstances under which the traffic accident occurred
- when one of the drivers refuses to give his/her personal or vehicle data
- when one of the drivers left the scene of a traffic accident
- the driver himself is the only participant in the traffic accident
- Do not leave the site of the traffic accident until police arrive and if possible take a photo of the damages of the vehicles and of the place of the traffic accident
- Exchange personal and insurance data with other participants, and in the case of a vehicle with foreign registration plates, provide a copy of the green card.
In case of a traffic accident with insignificant material damage when none of the aforementioned cases are concerned, the european accident report is to be filled download
PROCEDURE IN CASE OF A TRAFFIC ACCIDENT ABROAD
- Do not remove the vehicle from the road
- Place the special sign to mark a stopped vehicle on the road or any other appropriate signs in other to inform the participants in the traffic for the occurred danger
- Take photos of the accident scene and vehicles involved in the accident

- In case of an accident involving severe material damage, bodily or fatal /death consequences, be sure to report this event to the police in order to inspect the site and make a police report.
- Exchange copies of green cards with the other participant and / or give them to the police
- Ask for a Police report
- Fill in the report if the police require that from you and be objective
Instruction
Instruction for filling and usage of the european traffic accident report
- The participants in the traffic accident need to fill in and sign the European Accident Report, which must be in the vehicle
- Fill in correctly and sign it
- Provide accurate vehicle data (vehicle registration number and brand name), name and surname of the participant, his unique citizen number, insurance policy number and insurance policy validity period
- If there are witnesses, provide details (write down their names and surnames, address and telephone number)
- Report the damage personally to the company where the driver's vehicle is insured in the presence of the other participant involved in the accident, the vehicles and the completed European accident report
- It is advisable to report the damage immediately or within 3 days from the day of the accident
- Do not modify or repair anything on the damaged vehicle until the event is assessed and reconstructed.
